Dear Parents/Guardians,  This is to inform you of the upcoming Immunization Campaign targeting Grade 9 students for the school year 2014-2015.All grade 9 students are eligible for a vaccine against meningococcal infection. This vaccine offers protection against four types of meningococcal disease: A, C, Y and W-135. You will be receive a pamphlet shortly that will include the consent form for your child.Please complete both sections of the consent forms and return to your child’s homeroom teacher by Thursday September 11, 2014.The immunization clinic will be held at James M. Hill on September 29, 2014.Vaccines will not be provided without parental signature on the immunization consent form.
